You connect your own Discord application, so the panel carries your name and your avatar. Set the title, the description, the colour, the button label and the image, then pick the role a member gets when they come back from Discord.
The preview updates as you type, so you post the panel already knowing what it looks like.

Retract reads the connecting IP for VPN, proxy and Tor, checks whether the account shares a device or a network with an existing member, reads how old the Discord account is, and applies your country rules.

Yes. You create a Discord application and paste its token and client secret into Retract, which means the panel and the DMs carry your name and your avatar, not ours. Both values are encrypted at rest and never shown again.
Your member list is not in the server, it is in Retract. Make a new server, invite the same bot, and run a pull. Everyone who verified is added back with the role you choose.
No. The authorization they gave when they first verified is what Retract uses to add them back, so a restore needs nothing from them.
The Discord user id, the authorization token, and what you enable: the connecting IP for screening, the account age, and the answer to the lead source question. Tokens and bot credentials are encrypted at rest.
